Agent death quotient?

So I read that a Thieve's guild means loss of 3 levels, not 4-6 for unsuccessful missions.

Is that accurate? I have had plenty of agents die conducting missions based from thieve's guilds, losing way more than 4-6 levels.

It seems to me that missions against PCs or emmies are subject to that loss limit, but missions against groups (like assassinate wizards) are not- i.e. you fail, you die. Is this accurate?

Am I missing something explaining this in the rules? Or is my experience inaccurate (perhaps my memory fails)?

It definitely seems like with the most risky missions you can still die and not just lose 6 levels.  I have had the same experience.  I think you have to assume that is you fail an assassination you are toast.  Even with a thief's guild.
